Liability Waiver

Welcome to Activate Ministries Kids Ministry! We’re excited to have your child join us for a time of learning, fun, and community. As part of our commitment to providing a safe environment for all participants, we ask that parents/guardians carefully read and acknowledge the following disclosure regarding liability and responsibility.

By enrolling your child in Activate Ministries’ Kids Ministry, you acknowledge and agree to the following:

Voluntary Participation: Participation in our Kids Ministry programs is voluntary. By choosing to enroll your child, you understand and accept the inherent risks associated with activities, games, and events organized by Activate Ministries.

Supervision: While every effort is made to ensure the safety and well-being of all children in our care, you understand that it is not possible to monitor every child’s actions at all times. Our staff and volunteers strive to provide adequate supervision, but they cannot guarantee that accidents or injuries will not occur.

Assumption of Risk: You acknowledge that activities within our Kids Ministry may involve physical exertion, use of equipment, and interaction with other children. You understand that there is a risk of injury associated with these activities, including but not limited to bruises, cuts, scrapes, sprains, or more serious injuries.

Release of Liability: As a condition of your child’s participation, you hereby release Activate Ministries, its staff, volunteers, and affiliates from any and all liability for injuries, damages, or losses that may occur as a result of your child’s participation in our Kids Ministry programs. This includes, but is not limited to, injuries sustained during transportation to and from events, as well as injuries occurring on the premises of Activate Ministries.

Emergency Medical Treatment: In the event that your child requires medical attention due to illness or injury while participating in Activate Ministries Kids Ministry activities, you authorize the staff and volunteers to seek and consent to emergency medical treatment as deemed necessary. You agree to assume responsibility for any costs incurred as a result of such treatment.
